If you prefer not to use the on-line ordering shopping cart system, we have prepared a couple of fill-in forms for mailing or faxing your order. You have the options of payment by CREDIT CARDS, PERSONAL CHECK, MONEY ORDER OR PAYPAL. If using PayPal, enter your email and we will email you an invoice requesting payment.
For orders with DIFFERENT SHIP TO and BILL TO address, use this html form. This form will not calculate totals. You must enter all information manually.
For orders with SAME SHIP TO and BILL TO address, use the pdf form. This form will calculate totals for merchandise quantity and price. You will have to add the shipping charge based on the chart provided with the form. North Carolina residents must also add the 7% sales tax.
